To help ensure a safe, respectful, and engaging experience for all participants, all scholars and parents/guardians must review and acknowledge the Project Fibonacci STEAM Leadership Conference Code of Conduct prior to attendance.

The Medical Release Form provides Project Fibonacci staff with important health, emergency contact, medication, and treatment authorization information needed to support scholar safety throughout the conference.
Because scholars participate in workshops, off-site visits, transportation, and other conference activities, completion of this form is required for attendance. Parents/guardians should carefully review all questions and provide complete and accurate information before submitting.
